This position is responsible for ensuring the delivery of high-quality, patient-centered care while maintaining compliance with agency policies, applicable state and federal regulations, Medicare requirements, and established standards of clinical practice. The DON oversees clinical operations, coordinates patient care and staffing, supports and supervises clinical personnel, reviews plans of care and clinical documentation, and works closely with the Administrator and interdisciplinary team to promote quality outcomes and efficient agency operations. Essential Duties & Responsibilities Provide overall direction, coordination, management, and supervision of nursing and professional clinical services. Oversee the delivery and coordination of patient care to ensure services are appropriate, timely, and consistent with each patient's individualized plan of care. Review plans of treatment, clinical and progress notes, patient evaluations, case conferences, and ongoing patient-care needs. Ensure patient needs are continually assessed and plans of care are developed, implemented, reviewed, and updated appropriately. Make and oversee patient and clinical personnel assignments and ensure appropriate staffing and coverage. Coordinate referrals and communication between physicians, patients and families, clinical staff, payers, and other healthcare providers. Recruit, orient, train, supervise, evaluate, and support clinical staff in accordance with agency policies. Ensure clinical staff receive timely and properly documented orientation, education, and ongoing training. Monitor clinical staff performance and implement corrective action or remediation when necessary. Develop, review, revise, and implement clinical policies, procedures, standards of care, goals, and objectives. Maintain oversight of the agency's Performance Improvement/Quality Assessment (PI/QA) program and participate in quality-improvement projects. Identify clinical trends, deficiencies, and opportunities for improvement and implement appropriate corrective measures. Maintain knowledge of Medicare, Medicaid, third-party payer requirements, OASIS requirements, and applicable home health regulations. Ensure compliance with federal, state, local, and accrediting organization requirements. Promote compliance with infection prevention, emergency preparedness, OSHA, patient safety, confidentiality, and other clinical policies. Collaborate with the Administrator regarding clinical operations, staffing needs, resource allocation, and agency performance. Maintain effective communication with physicians, referral sources, patients, families, staff, and other members of the healthcare team. Participate in management, quality, and clinical meetings as required. Maintain a culture of patient safety, clinical excellence, accountability, and continuous quality improvement. Perform additional clinical or administrative responsibilities as necessary to support agency operations. These responsibilities closely reflect your existing DCS/DON specifications, particularly the requirements for patient assignments, coordination of care and referrals, continual assessment of patient needs, and oversight of individualized plans of care.

Qualifications Required:

Current, active Registered Nurse (RN) license in the applicable state. Nursing experience with demonstrated clinical competency. Home health or community health experience. Supervisory and/or administrative nursing experience. Strong knowledge of home health clinical operations and standards of care. Knowledge of applicable federal and state home health regulations. Understanding of Medicare requirements and the OASIS data collection/reporting process. Current

BLS/BCLS

certification. Strong leadership, organizational, communication, and problem-solving skills. Ability to effectively supervise and support a multidisciplinary clinical team.

Pay:

$85,000.00 - $95,000.00 per year
