The Wound Care Director will work closely with nursing leadership, physicians, nurse practitioners, therapists, dietitians, and facility staff to ensure high-quality, evidence-based wound care and positive resident outcomes. Key Responsibilities Oversee and coordinate the facility's wound care program. Conduct comprehensive wound assessments and develop individualized treatment plans. Monitor wound healing and revise interventions as clinically appropriate. Perform wound treatments and ensure care is provided according to physician orders and established protocols. Collaborate with physicians, advanced practice providers, nursing staff, dietitians, therapists, and other members of the interdisciplinary team. Identify residents at risk for skin breakdown and implement appropriate prevention strategies. Monitor pressure injuries, diabetic wounds, venous and arterial ulcers, and other skin integrity concerns. Ensure accurate and timely wound assessments, treatment records, and documentation. Educate nursing staff on wound prevention, treatment, documentation, and best practices. Monitor wound-related clinical outcomes and identify opportunities for improvement. Participate in care planning and interdisciplinary meetings related to wound care. Coordinate with outside wound care providers and specialists as needed. Ensure wound care practices comply with facility policies and applicable state and federal regulations. Maintain current knowledge of evidence-based wound care practices, products, and treatment approaches. Perform other duties as assigned by the Director of Nursing or Administrator. Qualifications Current Illinois RN or LPN license in good standing. Bachelor's degree in Nursing preferred. Minimum of 1 year of clinical nursing experience , preferably in skilled nursing, long-term care, or post-acute care. Previous wound care experience required. Experience overseeing or coordinating wound care programs strongly preferred. Wound care certification such as WCC, CWCN, CWS, or CWOCN preferred. Strong knowledge of pressure injuries, diabetic wounds, venous and arterial ulcers, skin integrity, and wound prevention. Strong clinical assessment, critical thinking, and problem-solving skills. Excellent communication and interpersonal skills. Ability to work effectively with residents, families, nursing staff, physicians, and interdisciplinary team members. Strong organizational skills and attention to detail.
