100% Onsite Position Summary We are seeking an experienced and highly motivated Director of Nursing (DON) to lead the clinical operations of our growing home health agency. The Director of Nursing will be responsible for overseeing all nursing services, ensuring compliance with federal, state, and accreditation standards, maintaining survey readiness, and driving quality patient care outcomes. The ideal candidate will have prior experience serving as a Director of Nursing in a home health setting and a strong track record of successfully managing state and federal surveys. This individual will serve as a key member of the leadership team and will play a critical role in supporting agency growth while maintaining the highest standards of clinical excellence and regulatory compliance. Essential Responsibilities Provide overall direction, supervision, and leadership for all clinical staff, including Registered Nurses, Licensed Practical Nurses, therapists, and other clinical personnel. Ensure the delivery of high-quality patient care in accordance with physician orders, agency policies, and regulatory requirements. Monitor clinical outcomes and implement performance improvement initiatives. Conduct regular case conferences and interdisciplinary team meetings. Support clinical staff development through coaching, education, and performance management. Regulatory Compliance & Survey Readiness Maintain continuous readiness for Illinois Department of Public Health (IDPH), CMS, and accreditation surveys. Lead all survey preparation activities and coordinate responses during surveys. Develop and implement corrective action plans when necessary. Ensure compliance with Conditions of Participation (CoPs), state regulations, Medicare guidelines, and agency policies. Monitor clinical documentation for regulatory compliance and quality standards. Quality Assurance & Performance Improvement Oversee the agency's Quality Assurance and Performance Improvement (QAPI) program. Analyze clinical quality indicators and implement strategies for improvement. Conduct audits of clinical records and documentation. Monitor infection control practices and patient safety initiatives. Operational Management Participate in strategic planning and agency growth initiatives. Collaborate with the Administrator on budgeting, staffing, and operational goals. Ensure adequate clinical staffing and effective scheduling practices. Assist with recruitment, onboarding, and retention of clinical personnel. Maintain productive relationships with referral sources, physicians, hospitals, and community partners. Qualifications Required Active and unrestricted Illinois Registered Nurse (RN) license. Minimum of 3 years of Director of Nursing experience in a Medicare-certified Home Health Agency. Demonstrated experience leading agencies through state, CMS, ACHC, CHAP, or Joint Commission surveys. Strong knowledge of Medicare Conditions of Participation, OASIS, PDGM, and home health regulations. Experience with quality assurance, clinical audits, and performance improvement programs. Excellent leadership, organizational, and communication skills. Proficiency with electronic medical records (EMR) systems and clinical documentation platforms. Ability to work onsite full-time in Mount Prospect, Illinois. Bachelor's degree in Nursing (BSN); Master's degree preferred. Home Health certification or advanced clinical leadership training. Experience with agency growth, start-up, or expansion initiatives.
