Job Summary:

Manages the day-to-day healthcare operations of the community to ensure resident's healthcare needs are met. Ensures residents are treated with respect and dignity and ensures quality care as resident's healthcare needs change. Supervises licensed nurses and other direct care staff within the community. Essential Functions Provides care to residents in accordance with physician orders, recognized standard of practice and established company policies and procedures. Responsible for direct supervision of community based licensed nursing staff, Med Tech/CMAs, Assist Resident Care Associate, Resident Care Coordinator, Resident Care LPN/RN Supervisor. Assists in maintaining a physical, social and psychological environment in the best interests of residents. Ensures resident rights and confidentiality are maintained at all times. Assigns and directs work of subordinates, appraising performance, rewarding, and disciplining associates, addressing complaints, and resolving problems. Assists on writing care plans based on resident needs and change of condition. Obtains and administers medication and treatments as prescribed by physician. Documents and reviews medication and treatment sheets for accuracy and compliance with physician orders, professional standards, federal and state regulations. Monitors responses to care treatment plans. Possesses knowledge of medication interactions. Adheres to company systems, processes and procedures. Counts controlled substances with off going nurse or designee; secures and corrects reconciliation of controlled substances during shift. Ensures cleanliness and organization of medicine supply room and security of discontinued medicine in accordance with federal and state regulations. Assists with discharge process relevant to medication and treatment orders. Possesses knowledge of OSHA regulations, infection control standards and emergency procedures. Follows policies and procedures to ensure resident safety. Documents all pertinent information regarding nursing care, care plans and observation of resident overall condition. Attends training to maintain skill level and remain current in emerging healthcare trends. Performs on-site evaluations of residents admitted to alternate care environments for treatment and maintains contact with residents' families at hospitals and skilled nursing facilities. Performs ongoing assessment/observation of residents physical and psycho-social needs and coordinates with other departments to assure quality proactive care (Quarterly and Yearly). Ensures that Staff is prepared for new admissions (Including pendants). Schedules Nursing staff according to facility requirements. Evaluates residents, documents change, and notifies executive director, physician, and resident's family as needed. Ensures residents are prepared for physician appointments, prepares Physician Visit Form, and reviews and updates resident's chart. Facilitates continuity of care for those residents receiving home health care, hospice services, or other third-party healthcare related services. Participates in or leads meetings relevant to resident care issues and participates in quality improvement activities. Ensures in-house ancillary medical services such as podiatrist, doctor visits, dental, psych, lab, etc. Assists in keeping the environment safe and evaluates resident transfer and mobility needs to reduce the occurrence of Worker's Compensation claims. Ensures that Staff is prepared for new admissions. Responsible for ensuring training of Nursing Staff and communication with BOM on new Nursing Staff. Follows up on Physicians Orders. Responsible for handling of Infection Control. Responsible for (pharmacy) approvals. Responsible for Nursing schedule. Responsible for clinics (flu, COVID) Responsible for reporting state reportable incidents on falls, COVID reporting. Responsible for chart audits, skin assessments, discharge Logs, Care Conferences. Responsible for ordering supplies and remaining with the nursing budget. Ensure that the building remains within state compliance.
