Job Description

Reports To The Health & Wellness Director will report to the Executive Director. This position is an exempt, salaried position. Job Overview Oversee the on-going day-to-day operations of the health and wellness department. Participates in the planning, implementation, and evaluation of the resident care. Responsibilities and Duties Responsible for all areas of the Health & Wellness department (compliance, planning, assessments, directing, budget, staffing, hands-on care) in accordance with current state regulations and company policies and proceduresConduct thorough resident assessmentsAssure resident care needs are met and maintained according to physician orders, state regulation, and company policies and proceduresMonitor the progress and effectiveness of the resident's care plan through written progress notes and through verbal communication with residents and associatesEvaluate the health and wellness associates' skill competencies and overall performanceResponsible for infection control management and QAPIResponsible for reporting, monitoring and the surveillance of all communicable diseasesParticipate in QAPI and responsible for determining the root cause analysis of resident data/outcomesComplete nursing audits as needed (med carts/medication boxes/storage, e

) to ensure compliance to state regulations and company policies and proceduresImplement training programs and in-services for all associates in accordance with state regulationsProvide ongoing leadership to the Health & Wellness associates and ensure staffing levels and shift schedules are operating at the highest levelHiring, training, and mentoring Wellness Nurses, and Health and Wellness Team MembersCoordinate ancillary support services for residentsInform physician, family, and associates of changes in resident's condition and needsMaintain department within budgetary guidelines for staffing and suppliesAssist with nurse coverage, as neededHas 24/7 on-call responsibilitiesOccasional weekend coverage as Manager on DutyParticipate in and attend all in-service training as scheduledEnsure compliance with all Federal, State, and local regulations, as well as community policies and proceduresOther duties as assigned
