Schedule:
Monday-Friday, 8:00 AM - 5:00 PM, with scheduled on-call shifts Position Summary CNS is seeking an experienced, full-time Clinical Care Coordinator (CCC) to lead day-to-day clinical coordination for our Provo Home Health and Hospice teams. Similar to a charge RN, the CCC is the operational and clinical anchor of the office — responsible for the intake and triage of client referrals, coordination of clinical services and procedures, and supervision of the clinical staff who deliver care in the home every day. The CCC also serves as a resource and liaison for patients, staff, referral partners, and the community. This is a great fit for an RN who is ready to step beyond bedside care into a leadership role that combines clinical judgment, team supervision, and care coordination.
Compensation & Benefits Salary:
Starting at $75,000 - PLUS. We offer more hourly based on experience, education, and work history.
Mileage:
$0.57/mile reimbursement for qualified travel
On-Call:
Shift unit compensation + hourly , Participation in the Administrator On-Call (AOC) rotation as needed
Schedule/Hybrid:
The CCC is required to be on-site five days a week during the first year. Step into leadership — lead a clinical team while staying connected to patient care Mission-driven work with Utah's oldest nonprofit home health and hospice company Better work-life balance than facility-based shifts and patient ratios Strong clinical support from an experienced executive and clinical leadership team Meaningful impact on patients, families, and the clinicians who serve them Key Responsibilities Conduct intake and triage assessment of client referrals; coordinate timely admissions and start-of-care visits Coordinate clinical services and procedures across the multidisciplinary team — RNs, LPNs, CNAs, therapists, social workers, and chaplains Supervise clinical staff; provide clinical guidance, mentorship, and accountability for care delivered within each team member's scope of practice and licensure Manage the clinical schedule and visit assignments to ensure timely, patient-centered care Conduct and facilitate Interdisciplinary Team (IDT) meetings in accordance with CMS Conditions of Participation and Utah regulatory requirements Complete Medicare OASIS reviews; ensure documentation accuracy and compliance with CMS standards Support hospice billing accuracy by ensuring timely and complete clinical documentation Oversee patient care satisfaction surveys and lead follow-up on opportunities for improvement Provide ongoing clinical education and training for the team Serve as a liaison with long-term care facilities, hospitals, physician groups, and community partners Act as a resource person for patients, families, staff, and the broader community Participate in scheduled on-call rotation as part of the leadership team
Qualifications Required:
Active, unencumbered Utah Registered Nurse (RN) license Minimum 3 years of nursing experience Minimum 2 years of supervisory or management experience Current BLS certification (or ability to obtain prior to start) Valid Utah driver's license, reliable transportation, and current auto insurance
Preferred:
1+ year of home health, hospice, public health, or community health experience Working knowledge of Medicare
OASIS, CMS
Conditions of Participation, and IDT/IDG processes Spanish speaking a plus
Skills & Competencies:
Excellent organizational and time management skills Strong leadership ability with a track record of building and supporting clinical teams Thorough knowledge of community resources Strong interpersonal skills, including good telephone etiquette Functional use of PC and computer input devices; comfortable with EHR documentation Physical Requirements Primarily office-based with regular local travel within the Provo service area Ability to sit for extended periods during administrative and coordination tasks Ability to drive throughout the service area; valid driver's license and reliable transportation required Ability to lift and carry materials or equipment up to 25 lbs as needed; ability to assist with patient transfers when on-call or in the field Visual acuity sufficient for reviewing documentation, EHR platforms, and digital communication tools This job summary outlines the general nature and level of responsibilities and is not an exhaustive list of all duties or skills required.
