Job Description

Our senior living community is seeking an experienced and compassionate Licensed Practical Nurse (LPN) to serve as our Director of Health Services / Director of Nursing and provide leadership and oversight of the Wellness Department. The ideal candidate is a strong assisted living leader who combines nursing experience with excellent people leadership, sound judgment, organization, and a genuine commitment to resident-centered care. This key leadership role is responsible for overseeing day-to-day wellness operations, developing and supporting the care team, coordinating resident services, maintaining high standards of care and compliance, and partnering closely with community leadership, residents, families, and healthcare providers. Key Responsibilities Provide leadership and day-to-day oversight of the Wellness Department Lead, coach, schedule, and develop caregivers, Med Techs, and other wellness team members Build a positive, supportive, and accountable team culture Oversee resident care processes, assessments and service planning within applicable LPN scope and community practices Monitor resident needs and changes in condition and ensure appropriate follow-up, documentation, and communication Support residents with varying levels of care needs, including memory loss, dementia, cognitive changes, chronic conditions, and other age-related needs Oversee medication management processes, documentation, and follow-up Coordinate resident care and services with families, providers, pharmacies, home health, hospice, and other healthcare partners Coordinate with an RN when RN assessment, nurse delegation, or other RN-specific functions are required Monitor staffing, employee performance, training, and the overall quality and consistency of resident care Maintain effective communication with residents, families, employees, providers, and community leadership Support compliance with Washington State assisted living requirements, survey readiness, quality assurance, and continuous improvement initiatives Partner closely with the Executive Director and leadership team to support resident satisfaction and overall community operations What We're Looking For We are looking for a strong LPN leader who understands assisted living and knows how to build, develop, and support a high-performing Wellness team.
The ideal candidate demonstrates:
Strong leadership, coaching, and team-development skills Hands-on experience in assisted living, senior living, and/or memory care Ability to establish clear expectations and accountability while maintaining a supportive team culture Strong nursing judgment within LPN scope of practice Excellent communication, organization, and problem-solving skills Confidence working with residents, families, providers, employees, and leadership Experience supporting residents with dementia, memory loss, cognitive changes, and varying levels of care needs Knowledge of medication management and assisted living resident care processes A calm, responsive, and solutions-focused approach A strong commitment to resident dignity, safety, quality of life, and service Qualifications Active and unrestricted Licensed Practical Nurse (LPN) license in Washington State required Minimum of 2 years of assisted living or comparable senior living experience preferred Previous supervisory, management, or demonstrated leadership experience required Experience leading caregivers, Med Techs, CNAs, nurses, or other resident care staff preferred Working knowledge of Washington State assisted living regulations, including Chapter 388-78A WAC, strongly preferred Experience with medication management, resident assessments, service planning, documentation, and changes in condition Memory care and/or dementia care experience preferred Strong communication, organizational, and leadership skills Compensation $90,000 - $120,000 per year Compensation will be determined based on the candidate's nursing experience, assisted living background, leadership experience, and overall qualifications.
