This role oversees discharge planning, utilization review, and care transitions for patients recovering from complex conditions such as stroke, brain injury, and spinal cord injury. The director will mentor case managers, collaborate with physicians and interdisciplinary teams, ensure regulatory compliance, and drive process improvements that enhance outcomes, patient experience, and length-of-stay efficiency in a faith-based, mission-driven environment.
