This is an excellent opportunity for a hands-on rehabilitation leader who is passionate about clinical excellence, resident outcomes, staff development, and interdisciplinary collaboration. The Director of Rehabilitation will oversee the daily operations of the rehabilitation department while ensuring high-quality PT, OT, and ST services are provided in accordance with resident needs, physician orders, regulatory requirements, and facility standards. Key Responsibilities Lead and oversee the daily operations of the rehabilitation department. Manage, mentor, and develop PT, OT, SLP, and therapy support staff. Ensure appropriate evaluations, treatment plans, documentation, and discharge planning. Monitor resident progress and ensure therapy services are clinically appropriate and outcome-driven. Collaborate closely with nursing, physicians, social services, administration, and other members of the interdisciplinary team. Ensure compliance with CMS, state regulations, Medicare/Medicaid requirements, and facility policies. Monitor therapy utilization, productivity, clinical outcomes, and documentation. Participate in care plan meetings and provide strong clinical leadership. Assist with identifying opportunities to improve resident outcomes, functional independence, and quality of life. Support survey readiness and maintain a high standard of regulatory compliance. Develop and implement quality improvement initiatives within the rehabilitation department. Build strong relationships with residents, families, physicians, and facility leadership. Maintain appropriate staffing levels and assist with recruitment and retention of therapy professionals. Qualifications Current license in good standing as a PT, OT, or SLP preferred. Previous leadership or supervisory experience in a skilled nursing facility strongly preferred. Strong knowledge of Medicare Part A/Part B, PDPM, documentation requirements, and SNF regulations. Demonstrated ability to lead and develop a multidisciplinary rehabilitation team. Excellent communication, organizational, and problem-solving skills. Strong clinical judgment and understanding of geriatric rehabilitation. Ability to work collaboratively with nursing and interdisciplinary leadership. Commitment to resident-centered care and achieving measurable clinical outcomes.
