Job Description

Corporate Director of Rehabilitation Services Multi-State Skilled Nursing Operator | Texas & Florida Hybrid Role - Houston, Texas or West Coast Florida We are seeking an experienced Corporate Director of Rehabilitation Services to provide leadership and oversight for therapy operations across a multi-state portfolio of skilled nursing communities in Texas and Florida. Our organization operates inpatient therapy programs across 16 SNF communities. This position will serve as the senior leader responsible for the overall performance, quality, compliance, and operational effectiveness of our rehabilitation services. This is a highly visible leadership role for an experienced therapy professional who understands both the clinical and operational sides of rehabilitation services in the skilled nursing environment. Key Responsibilities The Corporate Director of Rehabilitation Services will provide strategic and day-to-day oversight of therapy operations across all communities, including:
  • Provide leadership and operational oversight for PT, OT, and SLP services across 16 skilled nursing communities.
  • Lead and support facility-level therapy directors and rehabilitation teams.
  • Oversee therapy department staffing, recruitment, retention, scheduling, and workforce planning.
  • Establish and monitor productivity, utilization, labor, and operational performance expectations.
  • Develop, maintain, and implement rehabilitation policies, procedures, standards, and best practices across the organization.
  • Monitor therapy documentation, coding, billing practices, and clinical processes to promote regulatory compliance and appropriate reimbursement.
  • Maintain oversight of therapy-related compliance with applicable federal and state regulations, Medicare requirements, payer expectations, and company standards.
  • Develop and monitor clinical quality and performance metrics, identifying opportunities for improvement and implementing corrective action when appropriate.
  • Partner with facility Administrators, Directors of Nursing, regional operations leaders, and corporate leadership to support strong clinical and operational outcomes.
  • Assist communities with challenging staffing, productivity, compliance, or performance issues.
  • Support recruitment and development of therapy leadership throughout the organization.
  • Evaluate therapy delivery models, staffing structures, vendors, technology, and other opportunities to improve quality and efficiency.
  • Participate in internal audits, compliance reviews, quality initiatives, and development of corrective action plans.
  • Promote consistency and accountability while recognizing the operational needs of individual communities and markets.
Qualifications Qualified candidates should possess:
  • Current, active professional license as a Physical Therapist, Occupational Therapist, or Speech-Language Pathologist in Texas and/or Florida.
  • Significant experience providing rehabilitation services within the skilled nursing or post-acute care environment.
  • Prior experience leading therapy services across multiple facilities or locations.
  • Strong understanding of therapy department operations, including staffing, productivity management, clinical documentation, compliance, reimbursement, and quality.
  • Demonstrated ability to lead and develop facility-level therapy leaders and interdisciplinary teams.
  • Strong analytical and problem-solving skills with the ability to identify performance trends and implement practical solutions.
  • Excellent communication, organizational, and leadership skills.
  • Ability to work collaboratively with clinical, operational, compliance, financial, and executive leadership.
Experience overseeing therapy programs across a regional or multi-state skilled nursing portfolio is strongly preferred. Location & Travel This position is structured as a hybrid corporate leadership role, but it is not a fully remote position. The successful candidate should be based in or near either:
  • Houston, Texas, or
  • Crystal River, Florida The Corporate Director will be expected to maintain a regular on-site presence at one of these locations and travel routinely to communities throughout Texas and Florida.
Travel requirements will vary based on operational needs.
