Director of Therapy Duties:
Reviews, QA's, and approves all PT Evals and ReEvals, PT Discharges, PT Visits, and PTO Oasis documents and corrects the documents as necessary. Solicits information and works in conjunction with the supervising PT, patients, caregivers, and MDs as needed to obtain accurate and current data for each document Actively oversees all contract therapy delivered to HHP patients to ensure care plans are delivered appropriately Responsible for meeting all federal/state and local guidelines regarding therapy Responsible for developing the HHP HEP program that is delivered to all patients receiving physical therapy Supervises field staff as needed and per agency policy to ensure excellent patient care is delivered. Responsible for keeping track of timely patient documentation and communicates with field staff of their roles, responsibilities, and ethical duties for submitting timely and accurate charting Responsible for writing orders (as needed), communications, and other correspondence to ensure effective communication and excellent patient care Responsible for coordinating effective and appropriate labor utilization as needed for excellent patient care Works with other case managers, acting as Director of Physical Therapy, assisting them as and when needed Attends and actively participates in case conference Directs and keeps track of patient falls and provides fall prevention education and activities Assists with PT staffing/orientation as needed Provides and facilitates all education activities related to therapy Assists in patient satisfaction as needed Participates in a variety of HHP Teams About Home Health Professionals - Dallas Our team of experts provides care, treatment and services based on respect for our patients and passion for our job.
