Days | Occasional Weekends Job Summary Montello Health Care Center is seeking a compassionate and experienced Director of Rehabilitation to lead our rehabilitation services while providing direct patient and resident care. The Physical Therapist Director is responsible for overseeing the daily operations of the therapy department, ensuring high-quality clinical outcomes, regulatory compliance, and effective team leadership. This role combines hands-on patient care with departmental management and collaboration across interdisciplinary teams. Key Responsibilities Provide physical therapy evaluations, treatment planning, and therapeutic interventions for residents and patients. Direct and oversee the daily operations of the rehabilitation department. Supervise, mentor, and support therapy staff to ensure quality patient care and professional development. Ensure compliance with federal, state, and facility regulations, professional standards, and payer requirements. Coordinate interdisciplinary care planning with physicians, nursing staff, and other healthcare professionals. Monitor clinical outcomes and implement quality improvement initiatives. Manage department scheduling, staffing, productivity, and performance metrics. Maintain accurate and timely documentation in accordance with Medicare, Medicaid, and insurance requirements. Develop and implement outcome-driven treatment programs and rehabilitation services. Participate in budget planning, equipment management, and program development. Foster positive relationships with residents, families, referral sources, and team members. Utilize EMR systems and other technology to support clinical and operational efficiency. Qualifications Required Graduate of an accredited Physical Therapy program. Current Wisconsin Physical Therapist (PT) license in good standing. Minimum of three years of clinical physical therapy experience. Previous leadership, supervisory, or management experience preferred. Strong communication, organizational, and interpersonal skills. Knowledge of skilled nursing, rehabilitation, long-term care, and outpatient therapy practices. Proficiency with electronic medical record (EMR) systems. Must not be excluded or sanctioned from any state or federal healthcare program. Ability to meet the physical requirements of the position. Excellent written and verbal English communication skills. Preferred Experience in skilled nursing, long-term care, rehabilitation, or hospital settings. CPR certification. Experience with Medicare Part A, Medicare Part B, and managed care reimbursement systems.
