Job Description
Position Summary Under the general supervision of the Cardiology Practice Manager. The Cardiac Rehabilitation Coordinator is responsible for the coordination, delivery, and ongoing evaluation of the Cardiac Rehabilitation Program. This is a direct patient care and program coordination position that actively participates in the assessment, monitoring, education, and exercise supervision of patients enrolled in cardiac rehabilitation. The Coordinator provides hands-on clinical care, including cardiac rhythm/telemetry monitoring during exercise sessions, assessment of vital signs and patient response to activity, identification of abnormal findings or changes in patient condition, and appropriate clinical intervention and escalation. The position works collaboratively with physicians, advanced practice providers, nursing, rehabilitation staff, and other members of the interdisciplinary healthcare team to provide safe, evidence-based cardiac rehabilitation services and support optimal patient outcomes.
