Lynwood Key Responsibilities:
Conduct initial and ongoing strengths-based assessments to identify and prioritize the needs of children and families across all domains Provide field-based mental health and case management services to high-need, at-risk families Develop and implement individualized Family Support Plans in collaboration with multidisciplinary teams, families, and community supports Facilitate team meetings and ensure coordination and collaboration among all service providers, including DCFS, DMH, courts, schools, and healthcare partners Complete all required documentation, reports, and outcome measures in compliance with program guidelines and agency standards Provide crisis intervention and support as needed, participate in supervision and trainings, maintain ethical and licensing standards, and perform additional duties as assignedQualifications:
Master's degree in Social Work, Counseling, Psychology, Marriage and Family Therapy, or a related field required, with active CA licensure (LMFT, LCSW, LPCC) or registration (AMFT, APCC) and eligibility for licensure Minimum of one year experience working with children, adolescents, and families Experience with family-centered, strengths-based, field-based services and group facilitation Strong ability to collaborate effectively within a multidisciplinary team; experience working with diverse populations and knowledge of the local community preferred Fluency in Spanish highly preferred Valid California driver's license with a clean driving record requiredBenefits:
