SUMMARY:

Provide individual and/or family mental health assessment and treatment for children, adolescents, and their families in the school-based health center. May provide services to child and adult clients as requested at main sites.

SPECIFIC DUTIES AND RESPONSIBILITES

All duties are subject to accommodation in accordance with the Americans with Disabilities Act (ADA). Provides psychosocial assessment, treatment planning, therapy, and appropriate referrals to youth ages 5 through 18 (or upon completion of high school). Facilitates group sessions (e.g. CBITS/Bounce Back) as assigned. Refers clients for psychiatric care, Southwest services, and outside providers/services as necessary. Consults with supervisor, psychiatric providers, department members, and medical staff regarding client care. Participates in behavioral health child rounds, individual supervision, and department/agency meetings. Provides advocacy for clients within the school system (e.g. 504 and IEP meetings as requested by the client's guardian) and other systems such as DCF. Maintains client records in the EHR in accordance with SWCHC timeframes/policies and provides other documentation as requested. Performs other duties as assigned.

Requirements:

Qualifications:

Excellent organizational and communication skills. Computer literate including Microsoft Office; ability to document in an EHR. Ability to work well with culturally diverse populations. CPR/BLS Certified Education and/or

Experience:

Graduate degree in Social Work, Counseling or Human Services. CT License required.

LCSW, LPC, LMFT

preferred. Experience in providing services to medically underserved and culturally diverse populations. Ability to utilize Electronic Health Record (EHR)
