2989949271-2 Apply for this job now Job Description Oak Street Health seeks a Behavioral Health Specialist to provide integrated mental health services for adults on Medicare in underserved communities. In this role, you will conduct biopsychosocial assessments, develop person-centered treatment plans, and deliver evidence-based individual and group therapy. You'll collaborate closely with primary care providers, care managers, and social workers to support whole-person, preventative care. Bilingual Spanish skills are strongly preferred.
