JOB SUMMARY

Functions as part of a multi-discipline team in the delivery of clinical services to families with children, adolescents and young adults who are experiencing emotional, behavioral and/or mental health difficulties and who are at risk for out-of-home placement. Provides community-based services for clients and families that include ongoing clinical assessment, developing and implementing family-driven treatment plans and interventions, outcome measures, education and advocacy and crisis intervention services. Services are designed to support and sustain families in our community safely and avoid out of home care placements. Services will be provided through both face-to-face and telehealth services.

ESSENTIAL FUNCTIONS

Collaborates with team(s) to provide community-based clinical services to families experiencing primary issue with substance abuse or mental health. Implements therapeutic services, interventions, and modalities such as: comprehensive assessment crisis counseling individual counseling family counseling group sessions effective parenting substance abuse/co-occurring counseling family resource and support network planning, including direct linkage with formal/informal community supports. Delivers services in a variety of settings to include the family's natural environment, community centers and school. Services will also be conducted via Telehealth Services. Assesses risk factors during each contact and consults promptly with other team members and supervisor regarding any observations of an unsafe or at-risk situation. Accurately completes applicable documentation in the designated timeframes and ensures that all required documents have been received and signed by appropriate individuals. Participates in 24/7 crisis response, if applicable. Completes Clinical Service Plan/Treatment Plans that: is based upon the findings of the assessment, is developed in partnership with the family, describes the strengths and needs of the family, and desired outcome and discharge plan, contains issues, goals, and objectives that directly link to the assessment and are stated in clear, concise terms is culturally sensitive, and contains all appropriate signatures. Functions as a member of a multi-disciplinary team by: consulting with treating psychiatrist regarding progress participating in treatment team, team and/or community staffing, wraparound meetings other clinical meetings consulting with Team Manager and Director as well as other direct care staff regarding family progress, treatment and service planning, and discharge plans, as needed. Participates in weekly supervision with the assigned supervisor, requests supervisory input on cases as appropriate. Completes other duties as required by program or agency needs.

Requirements:

Demonstrates ability to operate basic office equipment, including fax, telephone system, laptop/tablet, and copier/scanner. Operates personal computer, demonstrating ability to perform basic Microsoft Office functions, Avatar, and required Electronic Record components and virtual meetings. Education and Experience:

Master's degree in behavioral health field with knowledge of mental health and substance-related diagnosis and treatment. Experience in mental health and/or behavioral health services which may include a master's level internship.

Physical Requirements:

Must be able to sit, twist, bend, stand, lift up to 30 pounds, and lower & raise self independently from the floor. Must be able to communicate orally and in writing. Must be able to complete NAPPI training and perform NAPPI techniques.

Special Working Conditions:

Exposure to consumers and a variety of program conditions, which may include long, irregular hours, on-call duties, weekend hours, and local travel. Exposure to a wide range of community environments outside the agency.
