2989949172-2 Apply for this job now Job Description Oak Street Health is seeking a Behavioral Health Specialist to provide high-quality, evidence-based care to adults on Medicare in an integrated primary care setting. In this role, you will conduct comprehensive behavioral health assessments, deliver short- and long-term therapy, and collaborate closely with primary care providers, nurses, and care teams to develop coordinated treatment plans. You will manage a diverse caseload, support patients with chronic and complex conditions, respond to crises, and contribute to quality improvement initiatives that advance patient-centered, preventative care in underserved communities. Responsibilities Conduct comprehensive behavioral health assessments and diagnostic evaluations for adult Medicare patients. Provide individual and group therapy using evidence-based modalities such as CBT and brief interventions. Develop and implement collaborative, patient-centered treatment and care plans with the primary care team. Monitor patient progress, adjust interventions, and document care thoroughly in the EHR. Coordinate care and referrals with internal and external providers, community resources, and social services. Respond to behavioral health crises, conduct risk assessments, and create safety plans. Participate in case conferences, huddles, and interdisciplinary team meetings. Contribute to quality improvement initiatives and population health efforts focused on behavioral health outcomes. Educate patients and caregivers on coping strategies, mental health conditions, and available supports. Maintain compliance with clinical, ethical, and regulatory standards, including confidentiality and documentation.
