2989949293-2 Apply for this job now Job Description Oak Street Health seeks a Behavioral Health Specialist to provide integrated mental health services for adult Medicare patients in underserved communities. In this bilingual English-Spanish role, you will conduct psychosocial assessments, deliver individual and group therapy, and create evidence-based treatment plans. Collaborating closely with primary care providers, nurses, and care teams, you'll support patients with chronic conditions, social stressors, and behavioral health needs. Responsibilities Provide individual and group psychotherapy to adult Medicare patients Conduct comprehensive psychosocial and behavioral health assessments Develop and update evidence-based treatment and care plans Collaborate with primary care providers and interdisciplinary care teams Deliver bilingual (English-Spanish) services and patient education Monitor progress, adjust interventions, and document in the EHRCoordinate referrals to community and social support resources Participate in case conferences, quality initiatives, and team meetings Support crisis intervention and risk assessment as needed Maintain compliance with ethical, legal, and regulatory standards Required Skills Clinical assessment and diagnosis Individual psychotherapy Group therapy facilitation Treatment planning and documentation Bilingual Spanish-English communication Crisis assessment and intervention Electronic health records (EHR) proficiency Care coordination and case management Motivational interviewing Knowledge of DSM-5 and evidence-based modalities
