Job Description

Job Duties:
Service Delivery Expectations:
1. Provides 5 hours per week group therapy per client and a minimum of 1 hour of individual therapy per assigned client (maximum caseload = 12). 2. Provides family therapy a minimum of 2 times per month and/or documents thoroughly any failed attempts to provide family therapy. 3. Provides didactic-educational groups as assigned by PACH Clinical Coordinator. 4. Completes intakes including Alcohol and Drug assessments on assigned clients within 8 hours of admission. Makes diagnostic recommendations. 5. Provides assessments, individual, group, and family therapy in compliance with DMH standards. 6. Facilitates referral of assigned clients for physician visits, lab work, and follow-up in compliance with program requirements and client needs. 7. Participates as a team member to provide a nurturing and consistent therapeutic treatment milieu. 8. Coordinates discharge planning and serves as liaison with referring and referral facilities. 9. Participates in residential activities and outings as assigned. 10. Provides program orientation to clients as assigned in a timely manner. 11. Transports clients as assigned in a safe and timely manner. Signs vehicle in and out per agency policy. Completes inspection prior to driving. 12. Reports any unusual incidents, safety concerns, or changes in client behavior indicating that the client may be a danger to self or others immediately to the Director of A&D Services or his/her designee. 13. Maintains client confidentiality at all times.
Documentation and Quality Improvement:
14. Participates in treatment plan reviews. Attends and participates appropriately in clinical staffings. 15. Documents appropriate activities in case note format that justify the need for treatment. 16. Completes and submits service logs, when applicable, per agency policy. 17. Maintains case record documentation that is adequate, appropriate, complete, and timely. 18. Responsible for DMH record compliance of all forms and documentation in case record of assigned clients.
Interpersonal and Leadership Skills:
19. Responds to clients needs in an appropriate and timely manner. 20. Demonstrates appropriate interactions with coworkers that promote a pleasant work environment. 21. Obtains shift report from departing coworkers. Provides shift report to oncoming coworkers that is complete and alerts oncoming shift of relevant concerns.
Support Duties:
22. Completes additional job duties as assigned. 23. Seeks professional certification and/or licensure as required (Program Therapist) and clinical training opportunities. 24. Participates in interagency collaboration for the provision of client services when assigned. 25. Notifies an appropriate designated supervisor a minimum of two hours in advance of any program absence so that the supervisor can arrange program coverage if necessary. 26. Monitors clients' belongings for items of safety concern and/or contraband. Removes any unsafe or inappropriate items and reports removal to Director of A&D Services or PACH Clinical Coordinator. 27. Identifies and reports to the Director of A&D Services in a timely manner any safety concern in the work environment. 28. Screens visitors and telephone calls to ensure only approved persons are contacting residents as per program policy. 29. Completes housekeeping duties assigned by Director of A&D Services in a timely manner.
