Job Description

Health Connect America, Inc. seeks a Licensed Mental Health Therapist to provide individual, family, and group therapy to children, adolescents, and adults in home, office, and community settings. Responsibilities include completing biopsychosocial assessments, developing person centered treatment plans, delivering evidence based interventions, and coordinating care with schools, agencies, and community partners. Role includes maintaining timely documentation, participating in multidisciplinary team meetings, and upholding ethical and cultural competence standards while supporting client safety, resilience, and long term well being. Responsibilities Provide individual, family, and group therapy to children, adolescents, and adults. Complete biopsychosocial assessments and diagnostic formulations. Develop and update person-centered, evidence-based treatment plans. Deliver in-home, in-office, and community-based therapeutic services. Conduct risk assessments and implement safety and crisis plans as needed. Maintain accurate and timely clinical documentation in EHR. Coordinate care with schools, agencies, and community partners. Participate in multidisciplinary team meetings and case consultations. Educate clients and families on coping skills, resources, and support options. Uphold ethical, legal, and cultural-competence standards in all clinical work.
