Full Job Description Purpose:

delivery of mental health services to individuals in need of counseling services. Provide in-home and/or in community individual and family therapy;

MUST BE ABLE TO PROVIDE IN PERSON SERVICES IN

one of the following counties: Gwinnett, Dekalb, Fulton, Cobb, Fulton, Henry, Spaulding or Douglas. Telehealth services can be provided, but some services MUST be in person.

Compensation:

Based on experience and credentials (Master level or licensed). Please note that this is an independent contractor position. As such, compensation will be issued via Form 1099

Duties and Responsibilities:

• Licensed Professional Therapist and/or S/T will perform individual and family therapy
• Licensed Professional Therapist and/or S/T will ensure documentation is inputted into Notenetic within 72 hours.
• Therapist will complete discharge summary on all assigned consumers

Requirements:

1. Master's Degree in Social Work, Psychology, or Counseling, - from an accredited university or college, and with documentation of supervised clinical experience in field placement. 2. Must have at least 1-4 years work experience in adolescent counseling and DSM-V diagnosis, to include crisis management, intensive case management, individual and/or family therapy, skill training, and other rehabilitation supports preferred for at-risk youth. 4.
