Job Description

The ROE 13-Child & Family Connections 21 office is accepting applications for a Service Coordinator position serving families in the Madison, Monroe, St Clair and Randolph County catchment area. Service Coordinator's welcome families and their children under age 3 years with developmental delays or medical diagnoses into the Early Intervention program and connect them with therapy providers including physical, occupational, developmental, behavior and speech therapists. Their role is to coordinate homebased Early Intervention services, support the family through the process of determining eligibility for therapy services, write Individualized Family Service Plans, coordinate ongoing services as well as to schedule and facilitate meetings. The CFC office offers a hybrid work model which requires highly organized, independent workers that are excellent communicators. Experience and comfort with Live Video Visit media platforms and computers is necessary. Individuals must have a valid drivers license and car insurance as this position requires travel to our families home's. Mileage reimbursement is provided. Service Coordinator's must hold a bachelors degree or higher in education, human services, behavioral science, social science or a health related field and must meet the credentialing requirements for Early Intervention Provider Connections. Additional Early Intervention training is offered.
