Our experienced team supports clients across a range of mental health challenges, ensuring they feel valued, heard, and empowered. We have two locations in Wolfeboro NH and Wakefield NH. Our practice can provide supervision for state licensure to a license eligible clinician. This position could be full or part time. Our private practice hires employees to be part of our work family, not independent contractors. Role Description This is a full-time or part-time hybrid therapist role for a Mental Health Counselor. Responsibilities include conducting individual and/or family therapy sessions, developing comprehensive treatment plans, and providing case management support as appropriate. The therapist will assess mental health needs, collaborate with clients to establish therapeutic goals, and maintain thorough and accurate documentation. Other duties include participating in team meetings, coordinating care with other professionals, and staying informed on best practices in mental health treatment. Qualifications Conduct comprehensive patient assessments to understand each person's unique needs, strengths, and challenges Develop and implement individualized treatment plans utilizing evidence-based therapies. Facilitate individual and family therapy sessions designed to promote emotional regulation, social skills, and coping strategies Collaborate with other healthcare professionals to coordinate care plans and provide guidance on strategies Collect and document clinical data accurately in compliance with HIPAA regulations and medical record standards Perform crisis intervention when necessary, ensuring safety and stability for clients experiencing acute distress or behavioral episodes Engage in ongoing case management activities including intake assessments, progress tracking, and discharge planning
