About us Our mission is to provide members of diverse communities with the highest quality of care by using Mental Health techniques designed to be Culturally Relevant, Family Focused, Client Centered, and Flexible. Commonwealth Mental Health and Wellness Center (CMHWC) is seeking experienced applicants for the role of a Therapeutic Mentor (TM). Qualified candidates will have experience in providing emotional support, mental health counseling, & working with youth under the age of 21.
Candidates with backgrounds in:
Psychology, Social Work, Counseling, Sociology, Education, and/or another Therapy/Mental Health related field of study are proffered. CMHWC offers care to clients of all ages, and service culturally diverse communities. Individuals fluent in additional languages are strongly encouraged to apply.
Responsibilities:
Provide Mental Health Services as a Therapeutic Mentor to youth below the age of 21 Aid in developing positive social and behavioral skills Work one-on-one with the youth, utilizing information from client's Guardian & In Home Therapy (IHT) Provider, in order to implement treatment goals Develop and implement age appropriate Lesson Plans with measurable outcomes Maintain client's mental health records, history, progress notes, assessments, disclosures, treatment plan etc. Ability to qork both independently and as a Team Maintain excellent communication between all parties of a client's Care Team Keep all client information confidential, according to the Health Insurance Portability and Accountability Act (HIPAA). Identify community resources needed to support and sustain achievement of client's treatment plan goals
