In this role, you will provide patient education, community outreach, care coordination, and wellness support to diverse populations. You will work closely with providers, nurses, social workers, and community partners to improve health outcomes and help patients overcome barriers to care. Qualifications Associate's Degree and at least 2 years of experience in health education, counseling, community outreach, patient education, or a related field. Bachelor's Degree in a healthcare, public health, education, or related field preferred. Bilingual communication skills in English and Spanish (written and verbal). Current Basic Life Support (BLS) certification. CPSP Certification or ability to obtain certification within one year of employment. Willingness to obtain certifications related to Health Education, Family Planning, and Chronic Disease Management. Strong presentation, communication, organizational, and relationship-building skills. Ability to work independently while collaborating with multidisciplinary healthcare teams. Understanding of cultural diversity, community resources, and underserved populations. Knowledge of preventive healthcare and patient-centered care models. Computer & Technology Skills Experience using Electronic Health Records (EHR/EMR) systems preferred.
