At True Health Center for Precision Medicine, our health coaches are an integral part of the clinical care team. You'll work alongside physicians, dietitians, and other health professionals helping patients turn sophisticated, highly personalized medical recommendations into changes they can actually make and sustain. A significant focus of this position will be our brain health and cognitive decline programs , where coaching can make an extraordinary difference. These patients often have complex care plans involving nutrition, exercise, sleep, stress, metabolic health, environmental factors, supplements, and other lifestyle interventions. They don't simply need another list of recommendations. They need someone who can help them understand what matters most, determine what is realistic, overcome barriers, and keep moving forward. You'll also have the opportunity to support and educate family members and care partners who are often essential to a patient's success. This is meaningful, relationship-driven work for a coach who wants to develop real expertise and become an important part of a highly collaborative precision-medicine team. What You'll Do You'll develop ongoing relationships with patients and help them translate their individualized care plans into everyday life.
Your responsibilities will include:
Conducting individual health-coaching sessions with patients, with a particular focus on brain health and cognitive decline. Helping patients prioritize and implement recommendations from their clinical care team. Coaching patients around nutrition, movement, sleep, stress management, healthy habits, and sustainable behavior change. Helping patients establish realistic goals and turn complex recommendations into manageable next steps. Identifying barriers to progress and creatively helping patients overcome them. Providing accountability, encouragement, and continuity between clinical visits. Supporting and educating family members and care partners when appropriate. Monitoring progress and communicating meaningful changes, challenges, or concerns to the clinical team. Collaborating closely with physicians, dietitians, health coaches, and other members of the care team. Documenting coaching encounters and patient progress accurately in the electronic medical record. Participating in patient education programs, group programs, workshops, and evolving practice initiatives. Are organized, dependable, detail-oriented, and comfortable managing multiple patients and priorities. Communicate well with clinicians and enjoy being part of a collaborative team. Are comfortable documenting care in an electronic medical record. Bring professionalism, good judgment, optimism, and appropriate boundaries to patient relationships. Qualifications We prefer candidates who have: Professional health-coaching, wellness-coaching, lifestyle-coaching, or comparable patient-support experience. Health Coach certification from a recognized program. National Board Certified Health & Wellness Coach (NBC-HWC) certification is a plus. Experience in functional, integrative, precision, or concierge medicine is highly desirable. Experience working with older adults, cognitive health, chronic illness, or complex patients is especially valuable. You do not need to arrive knowing everything we know about cognitive decline. We are looking for someone with a strong coaching foundation, intellectual curiosity, and a genuine desire to learn. We'll teach you our approach. Why True Health?
