The ITR model is one that promotes teamwork and dedication to the children, families, and individuals that we serve on a daily basis. The employee will find innovative ways to serve the population at hand. Participants that are involved with the ITR program are accustomed to a high level of care and may call upon you for advice or direction. The employee will provide Healthy Start Parenting Education & Support Services, they will participate in training to officially certify them to provide Healthy Start Services as a Community Educator with Inspire to Rise, Inc., if they are not already certified to teach the Healthy Start curriculum. The employee will perform services as requested by the participant and/or referral source; through the review of participant's records, additional information received from the referral source, the participant interview, and the assessment of the participant. These services can be provided in-home, at the Inspire to Rise Community Center campus, the Healthy Start Springfield campus, in jail, and/or in the community. The employee will perform Referral, Participant Assignment, Intake, and Pre-Assessment duties, including but not limited to the interview and pre-assessment of the participant, Healthy Start Assessment Tools as required, and/or other assessment tools as selected by Inspire to Rise, Inc. Executive Leadership Team. The employee will encourage participants to participate in all facilitated group and individualized classes. The employee will monitor participant's progress to evaluate success of services rendered, track progress, and adapt Service plans and/or Family Support Plan as required. The employee will complete a minimum of 2 home visits per month, per participant, that includes educational material from a Healthy Start pathway. In addition, a minimum of 2 care coordination calls per month are required. For a total of weekly contact with the family to insure engagement with the program.
