This role helps patients access PrEP/nPEP and other healthcare and supportive services while identifying and addressing barriers to care and treatment retention. Key Responsibilities Conduct HIV and HCV testing in office and mobile/community health settings. Perform sexual health risk assessments and determine appropriateness for PrEP. Provide education and prevention counseling related to HIV, HCV, STIs, and sexual health. Link patients to PrEP/nPEP, medical care, STI treatment, behavioral health, case management, and other supportive services. Assist patients with insurance enrollment, prior authorizations, and medication assistance programs. Identify and address barriers to healthcare access and treatment retention. Maintain timely, accurate documentation in electronic medical records while following HIPAA and applicable regulations. Collaborate with medical providers, case managers, and community partners to coordinate patient care. Maintain knowledge of community resources and services. Participate in required HIV prevention and health education training. Perform other related duties as assigned. Qualifications Bachelor's degree in public health, social services or related field OR equivalent experience working in the field. A minimum of 1 year of experience in the field and knowledge of HIV/STD infection, transmission and prevention; evidence-based interventions, with an understanding of health education principles and behaviors State certified HIV Rapid pre/post-test counselor within 60 days of hire. Phlebotomy certification preferred. Working knowledge of community resources and local referrals for HIV/HCV/STI and additional appropriate client-centered referrals preferred. Travel between New Port Richey and Port Richey locations required Valid Florida driver's license, proof of insurance, and reliable transportation Must pass Level II background check (https://info.flclearinghouse.com) and E-Verify employment eligibility (https://www.e-verify.gov/) Metro Healthy Communities is an equal opportunity employer.
