DUTIES/RESPONSIBILITIES
(include but are not limited to the following)
- Provide direct care, education, and counseling for inpatients and outpatients as directed by patients' physicians.
- Participate as a member of the multi-disciplinary team to develop, implement, and evaluate individual care plans.
- Document care and activities in the patient's medical record in a clear, complete, concise, and timely manner.
- Serve as a patient advocate to resolve concerns or complaints.
- Responds to appropriate medical emergencies.
- Assist in the development, and implementation of diabetes-related programs and materials for patients, staff, community, and other professionals.
- Participate as an active member in staff meetings and committees.
- Demonstrate effective work organization in stressful situations and as part of the diabetes team.
- Maintain awareness of financial issues, cost containment, and resources for patients, and is accountable for maintaining accuracy in the charging process.
- Maintain professional credentials and education necessary for professional and personal growth.
- Deliver education to patients following ADA standards of care
- Assist with downloading and placing supply orders for diabetes related technology
- Attend continuing education, staff development, and mandatory programs. Meets ADA requirements for diabetes continuing education.
