Short Description:
Provides programmatic and client support for the provision of patient-centered care . Complete intake assessment to determine unmet social services and healthcare needs; referral and linkage for medical care and social services; assist clients with healthcare and medication insurance applications and housing applications. Generalized knowledge of local and state resources for patient assistance programs for access to medical (Medicare and Medicaid), food, transportation and housing services. Encourage continuity of care between clinician and patient, by providing individual support and assistance to the patient and family in navigating the health care system. Demonstrate superior organizational & interpersonal skills; will be detail oriented; will have proven ability to work collaboratively, and be able to multi-task.
Experience Required:
Minimum of 3 years experience providing healthcare services and or community outreach for applicable populations Knowledge of health promotion and counseling preferred (specific disease and lifestyle related topics ideal). Knowledge of social services resources and requirments. Strong interpersonal skills; demonstrated ability to work well with people of various backgrounds, ethnicities, and life experiences.
Education Requirements:
Associates degree (preferably in health science) OR equivalent healthcare experience Bachelor's degree preferred.
License/Certification Requirements:
Applicable specialty certification(s) (i.e
EIP CSR-NMCM
;CDA/RDA, MA, CNA, LPN) Salary Min ($): USD $23.00 Salary Max ($): USD $37.00
