WORK ENVIRONMENTPotential for exposure to the hazards of the hospital environment, such as exposure to infectious disease, blood-borne pathogens, hazardous waste, and potential injury. This position requires considerable standing, reaching and walking. The ability to lift up to 50 lbs. is required.
Community position includes varies settings including but not limited to office, patients home, hospital, skilled nursing facilityPosition requires travel within the community. A personal vehicle is necessary, with mileage reimbursement provided according to company policy.

_____________________________________________ROLE DESCRIPTIONPOSITION SUMMARY
Community Setting Health CoachThe Health Coach participates as a member of the care team, working within the community, providing outreach to patients with complex medical and social needs. The Health coach acts as a resource to assist patients in navigating the health system; and serves to facilitate linkage to community resources under the direction of the Care Manager and LSW.The Health Coach collaborates with the multidisciplinary team in developing and implementing the care plan. This position engages patients by ways of telephonic coaching, home visit, hospital visit, and accompanying patients to provider appointments as needed. The Health Coach facilitates adherence to treatment plan by early identification and resolution of care gaps. This position provides appointment scheduling and other clerical duties as needed and assigned by program manager or designee.QUALIFICATIONSEDUCATION
High School Diploma required. Associates preferred.LICENSE/CERTIFICATION
AHA BLSHCP certification required within six months of hire or transfer. Valid NJ Drivers License required. Community Health Workers (CHWs) preferred.EXPERIENCE
1-2 years hospital clinical or physician office experience preferred. Proficiency in Clinical Applications preferred at time of hire; incumbents within position will be trained appropriately and then skill will be required for this position within 30-60 days from date of hire.PERFORMANCE EXPECTATIONS
