Job Summary Diabetes Care and Education Specialist
  • RDN serves in an advanced role providing in-depth analysis of the patient with diabetes and providing specialized care and in-depth education for persons with diabetes and their families.
Serves as a team member in assessing needs, planning and providing education and care, evaluating outcomes and coordinating follow up care for persons with diabetes. Job Duties Assesses patient's appropriateness for intensive insulin therapy. Provides diabetes self-management education and training which includes evaluation of the individual'sskills, support, and barriers to care. Assess general diabetes knowledge, evaluate physical findings and anthropometric measurements, Teach person appropriate essential self-care behaviors to improve effective self-management of their disease.

These may include healthy eating, being physically active, monitoring of blood sugar, medication administration and adherence, problem solving, healthy coping skills, and risk reduction behaviors. Developsdiabetes management plans appropriate to patient's immediate diabetes needs. Reviews and assesses lab results and adapts clinical recommendations and care plan accordingly. Serves as a resource for patient and family. Documents in accordance with established standards. Investigates and problem solves patient care issues withcare team and facilitates change to promote quality outcomes. Enhance quality of diabetes care through collaboration with other services and initiate referrals to appropriate support services. Works within the multidisciplinary team for diabetes education and management and collaborates with other disciplines to identify and resolve patient care issues. Conducts diabetes education through group classes as indicated. Participates in departmental initiatives to meet strategic goals. Precept students as indicated.

Mentor and train new staff members, colleagues, and other members of the care team as applicable. Practices with a high degree of autonomy Evaluates evidence-based best practice and shares back to team including proposing adjustments to supporting reference materials, policies, etc. Provides guidance to providers and other healthcare professionals. Participates in multi-disciplinary care team and functions as the diabetes expert Collaborates with inter/intra professional team members to achieve department goals and optimize service Serve as Designated Point Person for new hires as indicated. May include specialty program management and program evaluation outcomes Work is typically performed in a clinical environment.
