Full Job Description Now Hiring:

Healthcare Coach Part-Time or Full-Time Vita Forte Integrative Medicine is seeking a compassionate and motivated Healthcare Coach to join our growing team. The ideal candidate is passionate about integrative health and helping patients achieve their wellness goals through education, encouragement, and accountability.

Responsibilities:

Support patients in reaching their health and wellness goals Provide motivation, education, and ongoing accountability Assist patients with nutrition, exercise, sleep, stress management, and other lifestyle recommendations Track patient progress and document coaching interactions Communicate patient needs and concerns to the clinical team Coordinate follow-up appointments and encourage continuity of care Post approved educational videos and health information through the clinic's platform Collaborate with clinical providers to ensure educational content is accurate and appropriate Maintain patient confidentiality and comply with HIPAA requirements Help create a welcoming and positive patient experience

Qualifications:

Health coaching certification preferred Experience in healthcare, wellness, fitness, nutrition, or patient support is a plus Excellent communication and relationship-building skills Comfortable posting approved educational videos through online platforms Compassionate, dependable, organized, and self-motivated Comfortable working independently and as part of a clinical team Basic computer and electronic health record skills Commitment to professional boundaries and evidence-informed care
