Job Description
Community Health Centers of South Central Texas, Inc. (CHCSCT, Inc.), a FQHC (not for profit), is currently seeking a Full-Time Population Health Specialist - Gonzales location. Must be able to travel. Spanish a plus.
Primary duties will be:
Collaborate with internal and external stakeholders to identify population health priorities and develop strategies to address them. Contribute to grant development, management, and reporting activities related to population health and quality improvement. Collect, analyze, and interpret population health data to identify trends and areas for improvement. Develop and implement population health programs, interventions, and campaigns. Monitor and evaluate the effectiveness of population health programs and initiatives by gathering data and tracking relevant performance metrics. Work with various data reporting and population health tools. Provide education and training to staff on population health principles and best practices. Navigate patients through appropriate care paths and treat them with respect and dignity ensuring a positive patient experience. Serve as a resource and subject matter expert on population health programs as assigned. Stay current on emerging trends and best practices in population health.
