Job Description
Job Overview Function as a community health worker conducting no-medical drivers of health screenings, conducting home visits and delivering ongoing case management to connect participants with needed community and social services. Must have a valid Texas Driver's license and comply with AP 2-2 policy. Must be a certified community health worker or medical assistant with strong data entry skills using electronic health records, client record documentation, Microsoft intermediate skills, active listening and motivational interviewing skills.
