Administrator Ballard Center is seeking an organized and detail-oriented Health Information Management Coordinator/Director to oversee resident medical records and health information systems. This position ensures that records are accurate, complete, confidential, accessible, and maintained in compliance with HIPAA, federal and Washington State requirements, and facility policies. Primary Responsibilities Establish and maintain accurate electronic and paper medical records. Set up records for new admissions and close records following discharge, transfer, or death. Audit records for missing documentation, signatures, dates, physician orders, certifications, and recertifications. Notify responsible departments of deficiencies and track corrections through completion. Process authorized medical-record requests and releases within required timeframes. Maintain release-of-information and disclosure logs. Protect resident privacy and immediately report suspected confidentiality breaches. Scan, index, file, retrieve, archive, and securely store medical information. Assist with survey, audit, insurance, legal, and government record requests. Coordinate with Nursing, MDS, Therapy, Admissions, Billing, Social Services, providers, and facility leadership. Support documentation needed for ICD-10 coding, PDPM, Medicare, Medicaid, and managed-care reviews. Participate in survey readiness, staff education, and quality-improvement activities. Qualifications High school diploma or equivalent required. Knowledge of medical terminology, HIPAA, confidentiality, and electronic health records. Strong organizational, computer, communication, and time-management skills. Ability to work independently, protect confidential information, and follow assignments through completion. Previous medical-records or Health Information Management experience in a skilled nursing facility or healthcare setting preferred. PointClickCare experience and familiarity with SNF documentation, Medicare, Medicaid, ICD-10, and PDPM are preferred. RHIT, RHIA, or a related Health Information Management credential is preferred but not required. The successful candidate will be dependable, professional, accurate, and comfortable managing multiple deadlines while working collaboratively with clinical and administrative teams.
