Key Responsibilities Assist individuals with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and meal preparation Administer medications following prescribed protocols and accurately document all medication-related activities Collect and record data related to progress and service outcomes Support implementation of individualized Plans of Care (POCs) Transport individuals to appointments, community activities, and recreational outings Promote independence, positive behaviors, and skill development Collaborate with families, case managers, and healthcare professionals Ensure the safety, dignity, and well-being of all participants Schedule 12-hour shifts (3-day work weeks with alternating Wednesdays) Day Shift Sunday-Tuesday plus every other Wednesday 6:45 AM
