Health Information Tech I Location:
Whittier, CA (Shannon Tower, Lower Level, HIM Department)
Schedule:
Day Shift, 5x8-Hour, Monday - Friday, 8:00 AM - 4:30 PM About the Role This position processes health information under the direction of the HIM Director or designated supervisor. Responsible for coordinating physician medical record completion and the quantitative analysis of all medical record patient types based on standards established by Title 22, CIHQ, Conditions of Participation, and the Medical Staff Rules and Regulations. Duties and Responsibilities Safeguards and preserves the confidentiality of patients' protected health information in accordance with State and Federal (HIPAA) regulatory requirements and organizational policies Ensures a safe patient environment and adherence to safety practices per policy Utilizes approved processes to resolve biophysical, psychological, educational, and environmental needs of patients/significant others when administering care, with consideration to age Notifies physicians of medical records requiring completion in accordance with Medical Staff Bylaws, Rules and Regulations, Title 22, CIHQ, and other applicable regulatory agencies; maintains documentation of notifications Administers medical staff guidelines pertaining to medical record completion uniformly and consistently; may perform daily counts of records pending completion; monitors unsigned and refused electronic orders, tasks, and documents Retrieves incomplete records and/or assists physicians one-on-one in completing records electronically Activates temporary suspension of medical staff privileges when records are not completed timely; communicates suspension information per department procedures; maintains documentation for mandated reporting and credentialing needs Analyzes and re-analyzes incomplete paper and electronic medical records to ensure completeness; updates chart tracking system to reflect current status Scans loose filing into the ChartMaxx System Utilizes ChartMaxx to accomplish deficiency analysis and reporting Adheres to daily productivity standards Oversees all incomplete medical record activities and functions Assists physicians with record completion issues and escalates when resolution cannot be achieved timely Completes incident reports for any potential compensable event identified during record review or completion Conducts record review functions with established criteria and provides data to the Director or designated supervisor Performs basic eScription monitoring, pending, and look-up functions Operates standard office equipment including photocopiers, fax machines, computers, scanners, and telephones Communicates effectively with customers of all ages Abides by and enforces all compliance requirements and policies, performing responsibilities in an ethical manner Required Qualifications Knowledgeable of Windows software 3-5 years of medical record experience in an acute care setting Previous experience with electronic health record applications Preferred Qualifications High School graduate or equivalent Knowledge of physician record completion and HIPAA Knowledge of medical terminology Specific Skills Needed Knowledge of medical record documents, physician chart completion, and chart deficiency analysis Basic keyboarding skills; typing speed of 35 WPM Knowledge of medical terminology and familiarity with computers Ability to categorize forms/documents within the medical record Detail-oriented and self-motivated Ability to withstand pressure of continual deadlines and variable work requirements Ability to concentrate and maintain accuracy despite frequent interruptions Courteous, tactful, and cooperative throughout the workday Ability to use standard office equipment including computers, photocopiers, fax, and scanners Knowledge of Title 22, CIHQ, Conditions of Participation, Medical Staff Bylaws, and Medical Staff Rules and Regulations Additional Details
