Job Description
Job Summary Join our dynamic team as a Qmap Swing Shift, where you'll play a vital role in providing exceptional care and support to individuals with diverse needs. This position offers an energetic environment focused on delivering compassionate, high-quality service during the evening hours. You will be responsible for managing patient care activities, ensuring safety, and maintaining accurate medical documentation. Responsibilities Deliver comprehensive care to residents or clients, including assistance with daily living activities such as mobility, hygiene, and nutrition. Monitor and record vital signs, medication administration, and other medical procedures using electronic health record (EHR) systems like Epic or Cerner. Assist with specialized care needs such as Alzheimer's, dementia, memory care, hospice, and senior care. Follow strict protocols for infection control, HIPAA compliance, and patient confidentiality while documenting medical records accurately. Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each person's needs. Support residents with catheter care. Respond promptly to emergencies or changes in condition, communicating effectively with healthcare professionals and emergency responders when necessary. Qualifications Prior experience working in assisted living facilities, nursing homes, hospitals, or home care environments is highly preferred. Experience caring for individuals with developmental disabilities or those requiring memory or dementia care is advantageous. Familiarity with medical procedures including medication administration, vital signs monitoring, catheter care, and anatomy principles. Strong understanding of HIPAA regulations and commitment to maintaining patient confidentiality at all times. Excellent caregiving skills combined with a compassionate attitude towards seniors and individuals with disabilities. Ability to work swing shifts reliably in a fast-paced environment while maintaining attention to detail and safety standards.
