In this role, you will perform comprehensive assessments, administer medications and treatments, coordinate chronic disease management, and provide patient and family education. You will collaborate closely with physicians and an interdisciplinary team to develop and implement care plans, support preventive care initiatives, and ensure accurate documentation in the electronic health record.
