Job Description

Job Summary We are seeking compassionate and dedicated Home Health Aides and Caregivers to provide exceptional in-home care to clients across diverse needs. In this vital role, you will deliver personalized assistance that promotes independence, comfort, and well-being. Your energetic approach and attention to detail will ensure clients receive the highest quality of care in their homes or community settings. This position offers an opportunity to make a meaningful difference in the lives of individuals requiring support with daily activities, medical needs, and emotional comfort. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts or wheelchairs Monitor patient health by observing vital signs, recognizing changes, and reporting findings accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Administer medications and manage treatments following medical documentation protocols while ensuring compliance with HIPAA regulations Support clients with specialized care needs such as Alzheimer's, dementia, memory care, hospice, or developmental disabilities Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe environment Document patient care activities thoroughly and accurately in medical records or care logs Provide emotional support and companionship to promote mental well-being and reduce feelings of isolation Experience Prior experience in home health care, assisted living facilities, nursing homes, or long-term care environments preferred Hands-on experience working with individuals with disabilities or special needs such as Alzheimer's or dementia care Familiarity with medical terminology, medication administration, and vital signs monitoring Knowledge of using EMR/EHR systems for documentation and patient records management Certification in CPR and first aid required; additional certifications such as catheter care or hospice care are a plus Experience working with seniors or in community-based home care settings highly valued Ability to handle medical equipment like Hoyer lifts safely and effectively Join our team to provide compassionate, energetic care that transforms lives!
