In this vital role, you will provide compassionate, high-quality care to individuals living in assisted living facility environment. Your expertise will support residents with diverse needs such as memory care, dementia, Alzheimer's, hospice, and disabilities. This position offers an exciting opportunity to make a meaningful difference in people's lives while utilizing your medical knowledge and caregiving skills in a dynamic, fast-paced environment. Duties Deliver personalized resident care by assisting with daily activities such as bathing, dressing, and mobility support. Monitor vital signs and observe resident conditions to ensure safety and well-being. Assist with Administration of medication Use electronic health record (ALIS) systems to document care activities accurately and efficiently. Collaborate with healthcare teams to develop and implement individualized care plans tailored to each patient's needs. Maintain a clean, safe environment by adhering to infection control protocols and safety procedures. Communicate effectively with residents, families, and team members to ensure clarity of care plans and progress updates. Requirements Prior experience in assisted living facilities, nursing homes, hospitals, or home health care is highly preferred. Experience working with individuals with developmental disabilities or cognitive impairments such as dementia or Alzheimer's disease. Familiarity with EMR/EHR systems like ALIS for accurate medical documentation. Strong understanding of HIPAA regulations to protect patient privacy and confidentiality. Excellent caregiving skills combined with compassion and patience for working with diverse populations including seniors and those requiring hospice or memory care. Valid certifications in CPR/First Aid are preferred; Flexibility to work all shifts including days, evenings, nights, weekends, and holidays as needed.
