Job Description

Job Summary Work Location:
Woodford, IL 61760 We are seeking a reliable, tech-savvy, compassionate Telehealth Facilitator to join our team to service as of now facilties in Peoria and Pontiac, and potentially more. In this role, you will be the bridge between our residents in nursing facilities and their healthcare providers. You will travel to various nursing homes, move from room to room with a company-provided iPad, and ensure that every patient is successfully connected to their telehealth appointment via Zoom or other video conferencing platforms.
Key Responsibilities Facility Travel:
Travel to assigned nursing and rehabilitation facilities daily according to schedule coordinated with the facility and the provider.
Patient Assistance:
Visit individual patient rooms to prepare them for their scheduled appointments.
Technical Setup:
Set up iPad devices, ensure strong Wi-Fi/cellular connection, and launch Zoom or telehealth applications.
Provider Coordination:
Facilitate the "hand-off" between the patient and the remote doctor or nurse practitioner.
Patient Comfort:
Assist patients (who may have limited mobility or technical knowledge) in positioning the iPad so they can clearly see and hear their provider.
Troubleshooting:
Resolve basic technical issues with audio, video, or connectivity on the spot.
Sanitization:
Follow strict infection control protocols by sanitizing the iPad and equipment between every patient visit.
Requirements & Qualifications Experience:
Previous experience in healthcare (CNA, Medical Assistant, or Patient Transport) or a customer-facing technical role is preferred.
Technical Skills:
Proficiency with iPads, Zoom, and mobile apps. Ability to troubleshoot basic connectivity issues.
Compassion:
Extreme patience and a friendly demeanor when working with elderly or medically fragile patients.
Reliability:
Must have a valid driver's license and reliable transportation for travel between facilities.
Professionalism:
Ability to work independently while maintaining professional communication with nursing facility staff and remote providers.
Compliance:
Must be willing to undergo a background check and provide proof of required vaccinations (as per nursing facility mandates).
