Job Description

About ApogeeCare ApogeeCare is a pioneering healthcare organization specializing in geriatric telehealth solutions designed to enhance the quality of emergency care for older adults. By providing specialty telehealth assessments in emergency departments (EDs), ApogeeCare reduces the risks associated with hospitalization, allowing patients to return home safely with age-friendly care plans. Our team is dedicated to reshaping geriatric medicine standards through compassionate, efficient, and effective care for our aging population. Position Overview We are seeking an experienced and highly motivated LPN to join our team. This role is in person in the ED, supporting ApogeeCare's telehealth services by providing direct assistance to remote clinicians during geriatric assessments. The role requires comfort working in a fast-paced, acute clinical environment alongside ED physicians, nurses, social workers, and support staff. In this role, you will facilitate telehealth consults, assist in identifying eligible patients, perform focused chart reviews, support geriatric assessments, and act as a trusted point of contact within the ED. This position requires in-person shifts in the ED and reports directly to the Head of Care Liaisons.
Key Responsibilities Telehealth Coordination:
Identify and prepare older adults (65+) eligible for ApogeeCare telehealth services. Introduce ApogeeCare's telehealth services to patients and families, complete intake and obtain informed consent for services. Collect focused geriatric history under the direction of remote clinicians, including cognitive, mobility and functional histories. Maintain accurate documentation within electronic health records (EHR) systems, such as Epic, and support clinicians by organizing patient documentation.
Technology and EHR Management:
Maintain proficiency with computers and troubleshoot technical issues, with support from remote IT services, as they arise. Utilize electronic health records (EHR) efficiently for documentation, patient history and other updates, ensuring all data aligns with organizational standards. Set up and manage telehealth audio and video equipment (e.g., camera, tablet, audio equipment), and provide support as needed during telehealth consultations.
Liaison and Partnership Development:
Act as ApogeeCare's representative in the ED, fostering strong partnerships with ED staff, physicians, nurses, social workers, and other support departments to coordinate age-friendly, patient-centered care. Serve as the main point of contact for ApogeeCare, ensuring seamless communication and collaboration between ApogeeCare's remote clinicians and the ED team. Facilitate connections with external vendors and community support services, including Assisted Living communities, Skilled Nursing Facilities, Adult Protective Services, and other agencies to coordinate post-discharge care and resources for patients. Required Qualifications Active LPN Licensure At least 2 years of experience as an LPN, with Emergency Medicine experience preferred. BLS certification required; additional geriatric care training is a plus. Strong interpersonal and communication skills, particularly with older adults, and ability to work effectively in a team environment. Proficiency in electronic health record systems (e.g., Epic) and familiarity with telehealth and video communication technology. Proficiency with computers, typing, and managing multiple computer programs at the same time. Willingness to troubleshoot and problem solve computer issues as they arise on their own. A proactive and self-directed approach in a fast-paced hospital environment. Willingness and ability to learn or expand knowledge of age-friendly care. Compensation and Benefits ApogeeCare offers competitive shift-based compensation. We support continuous professional development to stay current with geriatric and telehealth best practices. Working Conditions Light physical demands are expected, including assisting patients and managing telehealth equipment. Adherence to safety protocols is essential due to potential exposure to bloodborne pathogens. Shifts may require flexible scheduling, including nights and weekends. Join Our Team! If you are passionate about transforming geriatric care through innovative telehealth solutions, apply to ApogeeCare and make a difference in the lives of older adults. Together, let's shape the future of compassionate, patient-centered emergency care.
