Responsibilities Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support Monitor patient condition through observation of vital signs and overall well-being Support medication administration following proper medical documentation and protocols Utilize Electronic Health Record (EHR) systems to accurately record patient information and care activities Help with specialized care needs including Alzheimer's, dementia, memory care, and end-of-life support Safely transfer patients using patient transfer methods and equipment like Hoyer lifts or other assistive devices Maintain patient confidentiality in compliance with HIPAA regulations and home healthcare standards Perform light housekeeping duties such as cleaning, laundry, and meal preparation to ensure a safe and comfortable environment Assist with catheter care, wound management, and other medical procedures under supervision or direction of healthcare professionals Communicate effectively with clients and their families to ensure personalized care plans are followed Requirements Prior experience working with individuals with developmental disabilities, geriatrics, or in hospice or memory care settings preferred Knowledge of medical terminology, vital signs monitoring, medication administration, and basic life support (CPR/First Aid) certifications required or obtainable upon hire Experience using EMR/EHR systems for accurate medical documentation and record keeping Ability to lift heavy objects safely and perform physical tasks involved in patient transfer and mobility assistance Familiarity with home health care regulations and best practices for patient safety and confidentiality Strong communication skills to provide compassionate client interaction and team collaboration Must be dependable, detail-oriented, and committed to delivering high-quality patient care in a variety of settings Join us in providing exceptional home health care assistance that enriches lives through dedicated caregiving.
