Salary Range:
$44.27 - $71.09 per hour Compensation is based on experience. This position is represented by a collective bargaining agreement. Multiple openings may be available. Make an Impact Beyond the Bedside As an RN Care Coordinator, you'll play a critical role in helping patients navigate their healthcare journey. Partnering closely with physicians, nurses, social workers, patients, and families, you'll coordinate care across the continuum, advocate for patient needs, promote positive outcomes, and ensure appropriate utilization of healthcare resources. This role is ideal for an experienced nurse who enjoys collaboration, critical thinking, care planning, and helping patients achieve the best possible outcomes. What You'll Do Coordinate and oversee patient care plans throughout hospitalization and discharge planning. Collaborate with interdisciplinary teams to create individualized care strategies. Conduct comprehensive patient assessments, including clinical, psychosocial, environmental, and discharge needs. Advocate for patients and families while helping remove barriers to care. Ensure appropriate utilization of healthcare resources and levels of care. Facilitate communication between providers, patients, families, and community resources. Identify opportunities to improve patient outcomes, care transitions, and patient satisfaction. Support timely and effective discharge planning to promote successful recovery and continuity of care. Serve as a trusted resource and leader within the care management team. What You'll Bring Required Qualifications Graduate of an accredited School of Nursing. Current Registered Nurse (RN) license in the state of practice. Current Basic Life Support (BLS) certification. Minimum of three years of acute care clinical nursing experience. Preferred Qualifications Experience in utilization management, care coordination, case management, or quality management. Strong understanding of healthcare systems, discharge planning, and care transitions.
