Job Description
Overview This role enrolls patients into a remote patient monitoring program (Evolko HealthRADAR) and keeps them engaged after enrollment. Success looks like a growing patient roster with low drop-off and quick resolution of non-responsive cases. This is a fixed-term position tied to the current phase of the HealthRADAR rollout. The initial term is one month, including training. Training is paid from day one of the contract. Responsibilities Enrollment Meet with eligible patients to explain the program in plain language and answer questions Help patients download the Evolko App and walk through setup Help patients Bluetooth-pair devices and take their first reading Coordinate with clinic staff to identify eligible patients and schedule enrollment sessions around normal clinic flow Complete the Enrollment Checklist for all patients enrolled each day Patient Follow-Up Call enrolled patients on a regular cadence to confirm correct use of the Evolko HealthRADAR Answer basic troubleshooting questions and escalate technical issues to support rather than fixing hardware directly Track each patient's status (active, non-responsive) and help patients remain active on the platform Reporting Check in regularly with the Program Manager on weekly progress and priorities Flag anything blocking enrollment Qualifications Prior experience in healthcare administration, patient-facing customer service, or a call center environment preferred; entry-level candidates with strong interpersonal skills will also be considered Comfortable interacting with older patients, with the empathy and patience to walk them through a digital onboarding process Warm, patient-centered communication style Comfortable using a smartphone app and pairing devices via Bluetooth Organized enough to manage a growing patient list without things slipping Reliable with a consistent weekly schedule Bilingual (Spanish) candidates might be preferred. Transportation- Candidates must be able to provide their own transportation to the clinic. Background Check & Certification Requirements- Offer is contingent on a standard background check.
