Job Description

The following is a brief description of the QIDP's responsibilities. Upon hiring, full job description will be discussed. The QIDP is responsible for the organization and delivery of services in the facility. The QIDP coordinates with other Department Heads to see that all services are provided in accordance with State and, Federal regulations and corporate policies Zuha Homes operates multiple intermediate care facilities in the San Bernardino, Los Angeles and Orange County areas. All clients are adults and have an intellectual disability, including cerebral palsy, autism, seizure disorder, etc. Clients attend local day programs during the week, and the homes are staffed according to state regulations. The ultimate goal is to afford every client the opportunity to gain as much independence as possible. QIDP/Administrator position for 3 ICF group homes licensed by CDPH in Los Angeles County. This is a fulltime, exempt position. BA in Social Services or a related field required. Experience with individuals with developmental disabilities required.
Key Responsibilities:
Programming:
a. Coordinates development of the ISP by: b. Obtaining assessments according to each client's needs. c. Scheduling and conducting the interdisciplinary team conference. d. Obtaining the completed ISP document for the client record. e. Completing the individual daily program schedule to incorporate the ISP objectives. f. Training facility staff to implement the ISP as written. g. Develops program activities and a schedule based on the ISPs of the residents. Purchases supplies and equipment to implement the programs. h. Supervises staff to conduct the training program as written. i. Coordinates the facility program with the day program for each resident j. Develops and arranges for the recreation and community participation/ integration program. k. Supervising meal preparation, serving and clean up to insures compliance, with regulations and Corporate policies. l. Monitoring the purchase or delivery of food and supplies for dietary services to insure that quality and quantity meets client needs. m. Supervises maintenance of physical plant, housekeeping and laundry services including:
  • Case management oversight
  • Supervise management of ICF staff/manager
  • Staff scheduling, medical monitoring
  • COVID management
  • Individual Service Plan development
  • Documentation including Individual Service Plans and Incident Reporting
  • Recruitment, hiring and retention of staff
  • Respond to afterhours emergencies.
Minimum requirements : Bachelor's degree in a human service related field Fingerprint clearance through the Dept. of Justice Valid driver's license One year experience working with individuals with intellectual disabilities Able to pass required physical Proficiency in basic computer programs Experience as a supervisor
