Position Summary:
Serves as the primary liaison with health care providers to ensure clients receive comprehensive and timely medical care. Coordinates all aspects of client health, including primary care appointments, immunizations, medication administration, and ongoing medical needs. Ensures compliance with California Community Care Licensing (Title 22), Department of Health, and County Public Health Department requirements. Oversees medical documentation, monitors health outcomes, and provides guidance to staff on medical protocols to maintain a high standard of care. Ensure a safe, trauma-informed, and nurturing environment for clients.Essential Duties and Responsibilities:
To perform this job successfully, an individual must be able to perform the following satisfactorily; other duties may be assigned. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Coordinate team based medical care for the children placed in the center. Prepare, compile, submit, and maintain accurate medical records, files, forms, statistics, and additional relevant information in accord with agency policy, licensing and/or funding requirements. Review ORR portal for compliance with medical information upload requirements and ensure updates are input as needed for safety and timely discharge. As appropriate, provide sponsors, family members and clients with medial updates. Actively participate in medical and service-related resource development prior to children's discharge. Participate in planning, coordinating, implementing, and evaluating all medical needs of the children for designated social service program(s). Establish and maintain partnership with medical care providers. Provide documented information related to identified delays in the medical assessments. Attend scheduled meetings and participate in team decisions and operations to represent the medical needs of the children. Coordinate medical resources for all employees and resource family homes. Adhere to Child Abuse and reporting requirements. Must be relied upon regarding task completion and meeting deadlines without sacrificing accuracy, work quality, or customer service satisfaction. Attending meetings and training when needed or requested. Perform other duties as requested by the assigned supervisor and/or management.Supervisory Responsibilities:
Directly supervises employees in the Department. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Supervise, coaches, and mentors staff providing guidance to their work and learning including, when necessary, corrective/disciplinary action in a timely and professional manner.Competencies:
